![]() Hospice and Home Care (HHCJ) nurses provide skilled care to end-of-life patients in the familiar surroundings of their own home as well as care for patients that are home bound after surgery or injury, or are recovering from cardiac problems or stroke. Services are limited to Juneau. Currently, HHCJ extends a broad base of services to patients, families and the community that are targeted at improving end-of-life care and bereavement, including those who may not have insurance or another payer source.
Services include visits from any and all of the interdisciplinary team members (nurses, therapists, home health aide, social worker, chaplain, and volunteers), as directed by the patient's physician. Services include equipment loans (e.g., wheelchairs, walkers, crutches, bath benches, bedside commodes, canes) free of charge to anyone in need, a variety of bereavement services, and volunteer services (e.g., companionship, caregiver respite, therapeutic massage, delivered meals, construction tasks, errands, recording of life histories and more). |
