Case Management make it possible for seniors with physical or cognitive disabilities to remain at home and live as independently as possible.
Case Management assess needs, determine eligibility for programs and benefits, and secure and monitor services in the home and community.
Southeast Senior Services has Case Management programs in Juneau, Ketchikan, Saxman, Angoon, Hoonah, Metlakatla, and Yakutat.
